What is a Shared Medical Visit?
It's an office visit with your doctor which includes:
- An individual medical check-up
- Prescription review
- Health education in a group with 20-30 patients who have the same medical condition that you have
Why choose to be a part of a Shared Medical Visit?
It means more time:
- with your doctor
- to learn about your condition
- to learn from others
- to offer support to others
We are now scheduling patients for Shared Medical Visits in DIABETES
- A shared Medical Visit is your regular continuing care visit with your doctor to take care of your diabetes, so regular billing procedures apply.
- You are encouraged to bring a support person with you to the visit.
- You will get to spend about 1 1/2 hours with your doctor or another diabetes educator, along with others who have diabetes.
